§ 5.101 GENERAL PROVISIONS.
   (A)   For purposes of this chapter, LICENSE shall mean the written authority granted by the city to maintain, operate or conduct a business or commercial establishment, or engage in any trade, profession or occupation or conduct any activity or maintain any premises which are subject to regulation under the police power for which payment of a fee to the city may be required; and PERSON shall mean person, partnership, corporation or other association.
   (B)   Applications for all licenses required by this chapter shall be submitted in writing to the Clerk and contain the following information, unless specifically provided otherwise herein:
      (1)   The full legal name, date of birth and address of the applicant. If the applicant is a partnership or corporation, the full legal names and date of birth of all partners, officers and all persons with a financial interest in the business. Where there are more than ten stockholders in a company, the applicant shall only be required to name the three shareholders with the greatest financial interest in said company.
      (2)   The specific license desired or applied for, citing the applicable section of this chapter.
      (3)   The proposed location of the licensed activity and the full name and address of the landowner of the proposed location if the owner is not the license applicant.
      (4)   An address where legal service of process can be made upon the named licensee during normal business hours and/or as stated in Articles of Incorporation as filed with the Secretary of State.
      (5)   Such further information as is requested and needed for the proper guidance of city officials.
