§ 118.035  CONTENT OF FRANCHISE APPLICATIONS.
   Applications for a franchise (not applicable to franchise transfers or renewals) shall include at a minimum:
   (A)   A clear description of the identity of the applicant, including but not limited to the name of the applicant, the address of the applicant, the nature of the business entity the applicant owns or represents, and the name, location, and address of any other cable systems the applicant owns or is affiliated with.
   (B)   Evidence that all aspects of the applicant's cable system will comply with all applicable laws including but not limited to the chapter and the city zoning code;
   (C)   A map or maps of a scale of not less than 1 inch equaling 100 feet showing the precise geographic area for which applicant seeks a franchise (such as, the proposed franchise area). The applicant shall propose a franchise area with a perimeter defined by streets, to the extent that such delineation of the franchise area is feasible;
   (D)   Evidence that the applicant has applied or is prepared to apply for building and electrical code review and approval of the plans for construction of the cable system within the proposed franchise area;
   (E)   Evidence of financial responsibility in the form of a performance bond conforming to § 118.050 of this chapter;
   (F)   A schedule of construction, as established by the applicant and reflected upon the map provided pursuant to division (C) of this section, showing by a logical geographic progression which streets within the franchise area will be included in the applicant's construction activities within each calendar quarter during the period of construction; and
   (G)   Detailed plans and specifications for the cable system which is proposed by the applicant.
