2.3.809: AUTHORITY TO ADOPT FORMS:
The Chief of Police is hereby authorized to adopt and/or approve forms necessary to enforce the provisions of this part. All persons required to be licensed under this part shall provide information as required by these forms. All forms or other documents required for compliance with the provisions of this part shall be legible. (Ord. 84-131; Ord. 01-42)
