All applications for broadband service shall be made through the office of the City Clerk, and shall contain the name of the owner, the name of the street upon which the property fronts, the official street number assigned to the premises, and the signature of the applicant agreeing to conform to the rules and regulations that may be established by the city as conditions for the city providing and the applicant using broadband service from the city.
