Applications for licenses required by Section 840.01 must be filed with the Chief of Police, on a form furnished by the Village, and shall contain the following information:
(a) The name and address of the applicant;
(b) The name and address of the person, firm or corporation by whom the applicant is employed;
(c) The length of service of such applicant with such employer;
(d) The place of residence and nature of the employment of the applicant during the last preceding year;
(e) The nature or character of the goods, wares or merchandise to be offered by the applicant; and
(f) A personal description of the applicant.
