Each permit application shall state at least the following, and the applicant shall adhere to any commitment made:
A. The full name, street address, telephone number, e-mail address, and facsimile number, if any, of the applicant;
B. The name, street address, telephone number, e-mail address, and facsimile number, if any, of the person in charge of the solicitation of contributions;
C. The date when the solicitation of contributions is to be conducted;
D. The time the solicitation of contribution will begin and end;
E. The highway and specific place on which the solicitation of contribution will take place;
F. The approximate number of participants, who will participate in the solicitation of contribution;
G. The prior solicitation of contributions on highway, history of the applicant, including the number of permits previously applied for;
H. Whether the applicant and participants have comprehensive general liability and other applicable insurance coverage as to which the city can be added as a named insured;
I. Whether the applicant and participants are indigent or any other reason why any applicable fees or deposit requirement should be reduced or eliminated; and
J. Any other information required by the city clerk or chief of police to perform the duties described herein. (Ord. 3023 §7, 2001, eff. 7-1-2001)
