(A) All persons desiring a permit in order to make an opening in any street or sidewalk, as set forth in § 95.01, shall make written application therefor.
(B) The application shall show the location of the proposed opening, the purpose therefor, and the approximate number of square yards of surface to be cut. The application shall be accompanied by the fee required by § 95.03 of this chapter.
(C) The fee required for making any opening in any street or sidewalk shall be established by the Board and maintained on file at the town office.
