Each person subject to the provisions of this chapter shall present to the City Clerk-Treasurer an application showing the applicant’s name, business address, business telephone number, residence address, residence telephone number, and if a non-resident of the county, a statement setting forth any previous, unpardoned felony convictions. Forms for applications shall be provided by the City Clerk- Treasurer and paid for out of the General Fund.
