The financial disclosure statement shall be on a form provided by the local Ethics Commission. The financial disclosure statement shall be filed annually by all elected officers no later than February 1. Candidates, nominees, and/or appointees shall file their initial statement no later than 21 days after the date of filing, nomination, or appointment.
