§ 111.07 PROCEDURE FOR APPLICATION.
   Not less than three days prior to the issuance of any license by the Clerk-Treasurer, an applicant for a license, as a condition precedent to the issuance of such license, shall, in writing and on forms provided by the Clerk-Treasurer, file with the Clerk-Treasurer a sworn application with:
   (A)   A valid photo identification card;
   (B)   A permanent home address and full local address of the applicant;
   (C)   A brief description of the nature of the business and the goods to be sold by applicant;
   (D)   If employed, the name and the address of the employer, together with credentials establishing the exact relationship;
   (E)   Proof of a valid state retail merchant’s certificate and/or a valid county health permit (if applicable);
   (F)   The length of time for which the right to do business is desired;
   (G)   The manufacturer or producer of the goods or products proposed to be sold, the address of the principal place of business, and point of manufacture or production of such goods or products and the proposed method of delivery (if applicable);
   (H)   A statement as to whether or not the applicant has been convicted of any crime, misdemeanor or violation of any municipal ordinance, the nature of the offense and the punishment or penalty assessed.
