Application for such permit shall be made in writing on forms furnished by the city clerk's office. The application shall be accompanied by a sketch showing the proposed yard, parking, any shelter to be located thereon, street access, and any structures or possible fire hazards in close proximity to the yard. The applicant shall covenant to clean and clear the yard and remove all Christmas trees, debris and temporary shelters prior to January 1 of the year next following the year of the issuance of the permit.
