§ 112.04 APPLICATION FOR LICENSE.
   An application in writing shall be filed with the City Clerk for a license under this chapter. The application shall provide the following information upon a form to be provided by the City Clerk. An application and the required license fee shall be paid at the time of filing of such application to cover the costs of investigating the facts and issuance of the license:
   (A)   Applicant’s full name, age, permanent and local address, phone number, and e-mail address.
   (B)   A description of the applicant including date of birth, race, height, color of hair and eyes, place of birth, and driver’s license number.
   (C)   A description of the merchandise or service to be sold.
   (D)   The business name, address, telephone number and e-mail address.
   (E)   The starting date and duration of the proposed solicitation or sale, and the specific hours, for the solicitation or sale in compliance with the requirements of this chapter.
   (F)   The period of time the applicant has been engaged in the same or similar business, and the jurisdictions in which the applicant has previously conducted business in the last year.
   (G)   Whether the applicant has ever had a license or similar license from the City or any other jurisdiction suspended or revoked.
   (H)   A list of all felony and misdemeanor convictions including traffic violations for the last ten years. Further, the applicant shall sign a release to authorize the city, Chief of Police or his/her designee, to obtain the applicant’s driving record, criminal history, fingerprint set, and to photograph applicant.
   (I)   If the license sought is for a transient merchant, the address of the private property where the sale will be held, the name and address of the property owner or person in control of that property and a written statement from the such person that the applicant is authorized to use the property for a sale on the proposed date. Each transient merchant shall also include a description of the structure, tent, trailer or other configuration from which the sale will be conducted.
   (J)   The application must be accompanied by a copy of all required permits and licenses, including but not limited to retail sales tax permit issued by the Iowa Department of Revenue. and if appropriate a food establishment license issued by the Iowa Department of Inspection and Appeals or I.A.C. § 481-31.7 if a pushcart or mobile food unit.
