§ 91.21 LICENSE APPLICATION.
   (A)   License applications for dogs or cats are available at the Clinton Finance Department, at the Clinton Humane Society or at another city-designated animal welfare agency. The applicant shall provide:
      (1)   A current rabies certificate from a licensed veterinarian which contains the tag number and expiration date of the rabies vaccine, or an exemption letter from a veterinarian;
      (2)   Evidence the animal has or has not been spayed or neutered;
      (3)   Evidence that the animal has or has not been microchipped;
      (4)   Name, color, sex and breed of dog or cat;
      (5)   Name, address, phone number and date of birth for the owner.
   (B)   No license shall be issued to a person under the age of 18. The application must be made within 30 days of obtaining the animal, or at age of four months of the animal.
   (C)   Upon request of a first-time license, the Clinton City Clerk or the Humane Society or other city-designated animal welfare agency shall provide a copy of § 91.02.
