§ 115.12 INVESTIGATION OF VIOLATIONS.
   In the event the Clerk is apprised of information indicating that permittee is in violation of § 115.11 or any other provision of this chapter such that grounds for denial, suspension or revocation of a massage therapy business permit may exist, the Clerk shall cause an investigation of the grounds to be made by the appropriate city department or departments and shall advise the Council in writing of the results of the investigation and any actions taken as a result of said investigation. A letter stating the actions taken by the Clerk as a result of the investigation shall be mailed to permittee's home address and the address of the permitted premises via U.S. mail and shall contain a short summary of the investigation, the city's findings, any and all actions taken as a result of the investigation, and the permittee's right to appeal the decision pursuant to § 115.13.
