§ 114.19 APPLICATION FOR VEHICLE FOR HIRE DRIVER'S LICENSE.
   (A)   A person desiring a taxicab driver's license shall file with the City Clerk a signed and sworn application on forms provided by the city, containing the following information:
      (1)   The name and address of the applicant; addresses for the past five years; date of birth; race; height; color of hair and eyes; place of birth; social security number and driver's license number;
      (2)   The experience of the applicant in the transportation of passengers; including any previous cities in which the applicant has operated a cab;
      (3)   Whether the applicant's motor vehicle license has been suspended or revoked within the previous three years and if so the reasons therefor and the dates and periods of suspension or revocation; and
      (4)   A list of all felony and misdemeanor convictions including traffic violations for the past seven years.
   (B)   Upon the approval of an application for a vehicle for hire driver's license and payment of the license fee, the City Clerk shall issue a vehicle for hire driver's license to the applicant which shall bear the name, address, race, date of birth, signature and photograph of the applicant. The annual fee shall be $35 dollars, $15 of which is a non-refundable fee for the mandatory background check performed for each applicant and renewal applicant.
   (C)   Any license issued after the effective date of this chapter shall expire one year after the date of issuance. A person may renew a driver's license 30 days before its expiration date, however, the initial date of issuance will remain as its expiration date. A person shall not be considered to be driving without a license during a period of 30 days following the license expiration date.
   (D)   A duplicate license may be obtained upon the filing of a signed and sworn statement by the person that the original license was lost, stolen or destroyed and the payment of a five dollar fee.
