§ 158.21  PRELIMINARY PLAT CONTENTS.
   The preliminary plat must contain and be accompanied by the following information and documents and meet the following requirements before it can be considered filed for approval.
   (A)   The preliminary plat application shall:
      (1)   State the name, business addresses and telephone numbers of the owners, engineer and surveyor;
      (2)   Describe the legal ownership interest of all owners in the tract;
      (3)   Cover all of the applicant’s contiguous property;
      (4)   State the type of development;
      (5)   Be accompanied by a written memorandum from the Public Works Director stating that the city has received a copy of the preliminary plat, concurs in the preliminary illustration of water and sewer utilities and either objects to or approves of the proposed plat;
      (6)   State the source for water service;
      (7)   State the source for sewer service;
      (8)   Provide a full legal description of the property covered by the preliminary plat, including all of applicant’s contiguous property; and
      (9)   Be accompanied by a copy of all private covenants and restrictions applicable to the land.
   (B)   The preliminary plat map shall include the following:
      (1)   All the applicants contiguous property, even though it is intended by the applicant to file multiple final plats and install improvements for parts of the property by section or unit depicted in separate final plats;
      (2)   The proposed plan of subdivision, showing proposed streets, blocks, lots, alleys, easements, building lines, required yard adjacent to streets, parks and the like, with principle dimensions;
      (3)   The location, width and name of existing streets and any blocks, lots, alleys, easements, building lines and watercourses or other natural features in the area affected, with principal dimensions, and any other significant information on all sides for a distance of 200 feet;
      (4)   The names of proposed streets. Such names shall conform to the names of existing streets of which they may be or become extensions, but otherwise shall not duplicate or conflict with the recognized name of any other street located in the city or its extra territorial jurisdiction;
      (5)   The location of existing sewers, water and gas mains and other public utilities, if any;
      (6)   The proposed general plan for sewer, water, gas and other public utilities in sufficient detail to indicate size and location of lines and structures;
      (7)   Proposed general plan for storm water drainage sufficiently detailed to indicate the location of drainage ditches or structures and the direction of flow;
      (8)   Zoning district classification of the area being platted;
      (9)   The name of the proposed subdivision, north arrow, scale and date;
      (10)   The name of the owners and the engineer and/or surveyor;
      (11)   Typical cross-section of proposed street improvements; and
      (12)   The seal of the surveyor or engineer responsible for the preparation of the preliminary plat map.
