Each person occupying a site within a RV park shall provide the following information to the owner, manager, operator or person in charge of the RV park:
(A) Name;
(B) Full address of permanent residence;
(C) Automobile and recreational vehicle license plate number and the state in which each is registered;
(D) Photocopy of driver’s license of the primary tenant;
(E) The number or letter of the designated site being rented; and
(F) Date of arrival and departure.
