§ 53.27 INFORMATION STATEMENT AS TO INDUSTRIAL WASTE DISCHARGE.

   (A)   Where the discharge of industrial wastes will occur, a written statement on a standard form provided by the control authority shall be filed with the city.

   (B)   The statement shall contain the following information:

      (1)   Name and address of the applicant;

      (2)   Type of industry;

      (3)   Quantity of plant waste;

      (4)   Typical analysis of the waste;

      (5)   Type of pretreatment proposed if any; and

      (6)   Proposed point of discharge.
