4-2-5: APPLICATION FOR LICENSE:
   A.   All applications for a beer license shall be made in writing upon the form provided by the city and shall show the information required by chapter 1 of this title.
   B.   An application for a class E beer license shall also show the following information:
      1.   A description of the times, dates, location, estimated attendance, nature and purpose of the temporary special event;
      2.   A description of the areas in which the applicant proposes beer be stored, sold or dispensed, and consumed;
      3.   A signed consent form stating that authorized license inspectors will have unrestricted right to enter and inspect the premises during the temporary special event.
   C.   All applications for a beer license shall also include a copy of a BCI background check by the Utah department of public safety bureau, dated no more than sixty (60) days prior to the date of application. Application for a beer license shall constitute a waiver of disclosure of any criminal conviction or plea of nolo contendere (no contest) or pleas being held in abeyance that have not yet been dismissed and shall constitute voluntary consent for verification of the BCI background check(s) by the police department, the Utah bureau of criminal identification (BCI) and the federal bureau of investigation (FBI) for the purposes of any proceeding involving the business or employee license.
