§ 97.09 RECORDS OF BIRTHS AND DEATHS.
   It shall be the duty of all physicians, nurses and midwives practicing within the limits of the town to keep an accurate record of all births and deaths occurring under their supervision, and upon the last day of each month every physician, nurse or midwife shall file with the Town Physician a written report of the birth or death; the reports shall be prepared upon blanks furnished by the town, and in case of births shall give date of birth, sex of child and name of the parents thereof; and in case of deaths shall give the date of death and cause thereof, the name, age and occupation of the deceased.
