2-2-3: LICENSE APPLICATION:
   A.   Application; Form: Any person desiring to sell intoxicating liquor under an on-sale license, a wine license or an off-sale license shall make his verified application in writing to the Board of Commissioners upon a form prescribed by the Commissioner of Public Safety, and shall file the same with the County Auditor.
   B.   Appearance Before Board: A person desiring an on-sale license, wine license or an off-sale license shall appear personally before the Board of Commissioners with the application as hereinafter required, and shall provide the Board of Commissioners any and all other information that they shall request.
   C.   Required Information: Such application shall require that the following information be set forth:
      1.   The name, age, date and place of birth, and place of residence of the applicant.
      2.   The name of the manager, principal operator, or other person who will be conducting and/or managing the premises for which the application for license is sought, who shall be a County resident and shall be designated as the agent for service of legal process. Whenever there is a change in managers, the said change must be furnished in writing to the County Auditor to be filed with the application for license for that respective year.
      3.   The location of the premises upon which the applicant proposes to sell such liquor and an exact description of the particular place within the building structure where such sales are proposed.
      4.   Whether the applicant has ever been engaged in a similar business, and if so, the location thereof and the date when so engaged.
      5.   A copy of the inspection checklist that is required by the Department of Public Safety and all other forms required by the State, the United States, the County, or other authority.
      6.   A complete description of the proposed hours of operation, days of operation and nature of operation.
      7.   A complete list of any and all convictions relating to misdemeanor violations for the use or consumption of intoxicating liquor and all gross misdemeanor and felony convictions whatsoever. (Ord., 5-6-1986; amd. Ord., 2-14-1995; 2000 Code)
