A. A business license application form shall be provided by the City Manager, which includes the following information:
1. Business name, address, and phone number. The business email address may also be provided, if available;
2. Emergency contact information, including a phone number;
3. A description of the proposed type of business;
4. Number of employees or rental units, if applicable;
5. Building owner(s) name, address, and phone number. The owner email address may also be provided, if available; and
6. Any additional information determined by the City Manager as beneficial for the effective administration of business licenses. (Ord. 698, 8-5-2020, eff. 8-31-2020)
