An applicant for a license shall furnish the Utility Office with the following information:
(A) Full name and address of applicant;
(B) The location of the proposed residential sale;
(C) The date or dates upon which the sale shall be held; and
(D) An affidavit from the applicant or applicants to the effect that all goods to be sold at the residential sale are owned by the applicant and have not been obtained for the purpose of reselling them at the sale.
(E) If an estate sale is conducted by a company. verification that the applicant or the applicant’s employer is registered with an Oklahoma Sales Tax Permit through the Oklahoma Tax Commission or other proof that sales tax has been or is being paid on the items sold or to be sold: or proof that the applicant or the applicant’s employer is exempt from the payment of sales tax.
