1330.03 ADMINISTRATION OF REGISTRATION.
   (a)   Department. The City Building Department shall administer the construction contractor registration provisions. Contractors may commence work immediately upon filing a proper and completed application with payment of fee and all submittals, without further City approval.
   (b)   Advisory Board. Refer to Ordinance 12-102-99 creating the Advisory Board.
   (c)   Registration Forms. Forms and applications for all registrations and renewals shall be prepared, kept on file, and supplied as needed by the Building Department. Each registration issued shall bear the signature of the Building Inspector or his/her designate. All applicants applying for registration under this chapter shall furnish the following information on the registration application:
      (1)   If applicant is a corporation, full and accurate corporate name, date and place of incorporation, names and addresses of corporate officers and statutory agent, and federal and state tax identification numbers;
      (2)   If applicant is other than a corporation, owners’ or individuals’ name(s), address(es), date(s) of birth and social security number(s);
      (3)   If the applicant conducts business under a trade or fictitious name, or is a partnership, applicant must include the business’ full and complete trade name, home office business address, and the names of all persons doing business under such name;
      (4)   Copy of applicant’s current liability insurance in an amount not less than three hundred thousand dollars ($300,000.00);
      (5)   Copy of applicant’s State license issued under Ohio R.C. Chapter 4740, if required to licensed;
      (6)   Certificate showing applicant’s current coverage with the Ohio Bureau of Workers’ Compensation;
      (7)   Proof of registration with the municipal tax department; and
      (8)   Surety bond for all contractors who perform work as prime contractors on projects for which there is no other performance bond in place (such as on public projects), in the amount of ten thousand dollars ($10,000.00) payable upon default to the project owner, the condition of default to be failure to comply with Building Code requirements.
   (d)   Fee Payment and Disposition. The fee for any construction registration required shall be seventy-five dollars ($75.00) initial or renewal. All fees and charges for registrations shall be paid in full in advance at the time application is made. Except as otherwise specified by ordinance, all registration fees shall be collected through the Building Department and paid into the General Revenue Fund.
   (e)   Registration Term. All construction contractor registrations shall be annual registrations and shall terminate on December 31 of each year. The Building Department shall send out to each registered contractor a renewal application one month in advance of expiration to the contractor’s last mailing address of record in the contractor’s registration.
   (f)   Business Changes. All material business changes affecting a registered contractor, including business or home office location, name changes, or ownership change, shall be promptly reported to the Building Department.
   (g)   Posting Registration. It shall be the duty of any construction contractor operating in the City to make its registration available upon request, and to keep the registration posted at all times in a prominent public place on all premises used for such business, including any place where the general public might engage the contractor, but not including on the job site.
   (h)   Code Compliance. All work performed by any construction contractor shall be in compliance with the Ohio Basic Building Code or the Council of American Building Officials Code as well as the National Electric Code, the Ohio Plumbing Code, Ohio EPA requirements, or any other code applicable to the work.
