§ 152.035 PERMIT REQUIRED.
   (A)   It shall be unlawful for any person to dig up, break, excavate, tunnel, undermine, or in any manner breakup any street, or make or cause to be made any excavation in or under the surface of any street for any purpose, or to place, deposit, or leave upon any street any earth or other evacuated material obstructing or tending to interfere with the free use of the street, unless such person shall first obtain an excavation permit from the Town Clerk.
   (B)   The application for an excavation permit shall contain the following information:
      (1)   The name and address of the applicant;
      (2)   The purpose of the excavation;
      (3)   The location of the work to be done, and the name of the person or firm who is to do such work;
      (4)   The estimated cost of the proposed work; and
      (5)   The date of commencement and date of completion of the proposed work, and other dates and plans as may be required by the Town Clerk or the Town Services Superintendent.
   (C)   The Town Services Superintendent must approve all application before a permit may be issued.
