§ 152.021 CONTENTS OF APPLICATION, DEPOSIT; DRIVEWAYS AND CURBS.
   (A)   (1)   The application for a permit shall contain the following information:
         (a)   The name and address of the applicant;
         (b)   The dimensions of the work to be done;
         (c)   The location of the work to be done, and the name of the person or firm who is to do such work;
         (d)   The estimated cost of the work; and
         (e)   The date of commencement and date of completion of the proposed work, and other dates and plans as may be required by the Town Clerk or the Town Services Superintendent.
      (2)   The Superintendent of the Street, Sewer, and Water Department must approve all applications before a permit may be issued.

   (B)   Any applicant for a driveway permit must post with the Town Clerk a deposit, as set by resolution, payable to the town, to protect the town from any liability which may arise as a result of the work to be done. The Superintendent must inspect and approve all work performed on property before the deposit is refunded.
