§ 111.03 APPLICATION.
   (A)   Application for a license to engage in the business of operating a cable television system will be made by the person, or his or her duly designated agent, by filing such application with the Town Clerk.
   (B)   Such application shall contain the following information:
      (1)   The name and address of the applicant, and should the applicant be a corporation, the names and addresses of all officers and directors of said corporation;
      (2)   An outline of the general construction and operating plans of the applicant;
      (3)   A proposed schedule for the construction of the system and the commencement of the service, or, if the system is already operational, a full and complete description, including maps and plans of the system;
      (4)   A statement of the policy of the applicant concerning the extension of the system within the license area; and
      (5)   Such information as may be required to determine the legal character, financial, technical, and other qualifications of the applicant.
   (C)   Within 30 days following receipt of the application, the Town Clerk shall submit said application to the Town Council; the Town Council shall, at a regular or special meeting, adopt a resolution to either approve the application as submitted, approve the application subject to modifications, or deny said application.
