§ 36.03 SCHEDULE OF CHARGES AND FEES.
   (A)   Photocopying charges.
      (1)   (a)   Per page 8½" x 11", black and white         20 cents
         (b)   Per page 8½" x 11", color            40 cents
      (2)   (a)   Per page 8½" x 14", black and white         25 cents
         (b)   Per page 8½" x 14", color            50 cents
      (3)   (a)   Per page 11" x 17", black and white         35 cents
         (b)   Per page 11" x 17", color            70 cents
   (B)   Facsimile Transmission charges.
      (1)   Per page 8½" x 11" local receiving number         25 cents
      (2)   Per page 8½" x 14" local receiving number         25 cents
      (3)   Per page any size long distance            $1.00
   (C)   Certifications or acknowledgment of official records.
      (1)   For each public document               $1.00
      (2)   For each document not official public business       $2.00
   (D)   Other documents. Planning booklets, standards and other documents not listed above shall be as determined by the Clerk-Treasurer consistent with the Indiana Code and the town policy as defined in this subchapter.
   (E)   Reproduction costs and service fees charged by the Cicero Police and Fire Departments.
      (1)   Reproduction costs.

      (2)   Service fees and charges.  
         (a)   Fees and charges for such things as gun permits, non-resident motor checks, fingerprints, and the like, shall be in accordance with the fees and charges set forth in the Indiana State statutes.
         (b)   Golf cart registration. Registration fees for golf carts are as follows: annual permit, $30.00; seven-day permit, $15.00.
   (F)   Ambulance and medical service fees.
      (1)   The following user-fee schedule shall be charged for all ambulance and/or medical services provided by the Town of Cicero through its Clerk-Treasurer's Office:
         (a)   ALS emergency:

      (2)   The fees set forth in division (1) shall be charged per patient run occurrence; however, when more than two members of an immediate family residing at the same address are transported per incident, the basic fee, mileage fee, and advance life support fee shall be only billed for two persons.
      (3)   For billing purposes, the type of run and appropriate charge shall be determined by the records of the Cicero Fire Department.
