(A) The Building Commissioner shall issue a building movement permit when supplied with the following:
(1) The name of the applicant.
(2) The address of the building.
(3) The name of the owner of the land if different from applicant and such owner's consent to removal of the building.
(4) The name of the owner of the building if different from the applicant and such owner's consent to the removal.
(5) The address of the land to which the building will be moved.
(6) The name of the owner of the land to which the building will be moved and such owner's consent to the removal.
(7) The name, business address, and business telephone number of any person or firm which will be performing the removal and relocation.
(8) The date, not later than 30 days from the date of application, by which the building will have been removed from its foundation, moved to its new location, and permanently reattached to a new foundation.
(9) The consents of the Chief of Police, the head of the Street Department, and the consents of such public utilities, as will be affected by the removal, storage, and relocation of said building, and to the extent it is necessary to obtain permission of county or state highway authorities, written evidence of such permission.
(10) The posting of surety bond, in an amount not less than $5,000, binding an approved commercial surety to the town in the event of damage to the property of the town, including damage to street surfaces.
(11) The filing of proof insurance insuring the interests of the town, the applicant, the person or firm which will perform the removal and relocation, and any owner other than the applicant against liability for personal injury and property damage caused by such removal and relocation, in an amount not less than $100,000 per person and $300,000 per event.
(12) Inspection by the Building Commissioner of the proposed foundation or slab upon which the building will be placed and approval for such relocation if such relocation is within the corporation limits of the town or written evidence of such approval by the relevant planning authority, if any, in the jurisdiction into which the building will be moved.
(B) The permit issued under this chapter shall expire at midnight of the thirtieth day following its issuance without further notice. Any change in the status of the surety bond or liability insurance coverage shall void the permit.
