§ 95.04 AMENDMENT.

(A) Amended plans filed hereunder shall bear the title "HAZ-MAT Emergency Pre-Plan Amendment (year)." Amendments to an existing plan shall bear the title "HAZ-MAT Emergency Pre-Plan Amendment (year)." The original plan and any amended plan shall bear a face sheet containing the following: "The following is the HAZ-MAT Emergency Pre-Plan for the Town of Cicero, Indiana as approved this 8th day of September, 1987," and shall be followed by the signatures of the Chief of Police and the Fire Chief.

(B) Amendments to an existing plan shall bear a face sheet containing the following: "The following are amendments to the HAZ-MAT Emergency Pre-Plan (year) of the Town of Cicero, Indiana as adopted this 8th day of September, 1987," followed by the signatures of the Police Chief and the Fire Chief.
