5.40.130: RECORDS:
   A.   Each treatment clinic director shall obtain the following information and make it available for on site inspection upon request by the code enforcement officer, any police officer or other designee of the city:
      1.   Names and ages of children under care;
      2.   Names, addresses and telephone numbers of the parents or guardians;
      3.   Accounting of the days and times for which care is provided;
      4.   Copy of the child's birth certificate;
      5.   Medical treatment consent forms signed by the parent or guardian of a child under treatment;
      6.   List of medications taken by any child under treatment;
      7.   A list of all licensed or certified professional staff;
      8.   Documentation of all staff's CPR/first aid training, and twenty (20) hours of continuing education unit training in the field in which they are licensed.
This information shall be maintained in an orderly manner by the clinic. Failure to maintain and/or allow inspection of the above records shall be grounds for revocation of license. (Ord. 632 §2, 2007)
