§ 110.73 APPLICATION FOR LICENSE.
   (A)   Application for permission to make such calls shall be in writing, stating:
      (1)   The purpose of the intended call or calls;
      (2)   The names, addresses, and other identification of all persons who will be in charge of, and of all persons who will be making the calls;
      (3)   An outline of the method to be used in making the calls;
      (4)   The license numbers of all vehicles, if any, to be used in conveying the callers;
      (5)   The area and location of the dwellings to be called upon;
      (6)   The time or times when such calls will be made; and
      (7)   Such other information as may be required by the safety-service director or by his duly authorized agents or representatives, concerning the applicant, the organization, or the nature and purpose of the calls.
   (B)   The safety-service director shall cause a criminal background check to be conducted on each person listed on the application.
   (C)   Licenses shall be issued for a period of 30 days from the date of issuance but may be revoked by the safety-service director at any time provided herein. A charge of $150 shall be made for each person listed on the application. Payment must be made before the license is issued. Renewals of the license or licenses may be for an additional 30 days with payment of an additional $150 per person.
