Applications for licenses for distributors for amusement machines shall be obtained at the Cheviot Police Department. Applicants shall file separately as distributor or exhibitor. Applications shall be in triplicate and shall be approved by the chief of police. Application forms and licenses to be displayed on each amusement machine shall be prescribed by the chief of police.
