§ 116.11  DRIVER’S LICENSE.
   (A)   Any person driving a taxicab or motor vehicle for hire shall procure a license therefor.
   (B)   Each applicant for a driver's license shall:
      (1)   Be of the age of 18 years or older;
      (2)   Be of sound physical condition and good eyesight, and not subject to epilepsy, vertigo, heart trouble or any other infirmity of body or mind which might render him unfit for the safe operation of a public vehicle;
      (3)   Be able to read and write the English language;
      (4)   Be clean in dress and person, and not addicted to the use of intoxicating liquors;
      (5)   Fill out, upon a blank form to be procured from the Chief of Police/Director of Public Safety, a statement giving his full name, residence, place of residence for five years previous to moving to his present address, age, color, height, color of eyes and hair, place of birth, length of time he has resided in the city, whether a citizen of the United States, place of previous employment, whether married or single, whether he has ever been convicted of a felony or misdemeanor and, if convicted, the nature of the crime and the date and place of such conviction, whether he has previously been licensed as a driver or chauffeur and, if so, when and where and whether his license has ever been revoked and for what cause; which statement shall be signed and sworn to by the applicant and filed with the Chief of Police/Director of Public Safety as a permanent record.
   (C)   Each applicant for a driver's license shall be examined by the Chief of Police/Director of Public Safety as to his knowledge of the provisions of this chapter, the traffic regulations and geography of the city. If the result of the examination be unsatisfactory, he shall be refused a license. Each such applicant shall, if required by the Chief of Police/Director of Public Safety, demonstrate his skill and ability to safely handle a public vehicle by driving it through a crowded section of the city accompanied by the Chief of Police/Director of Public Safety.
   (D)   Any person holding a public driver's license shall make application for the renewal thereof upon forms to be furnished by the City Clerk, entitled “Application for Renewal of License,” which shall be filled out with the full name and address of the applicant, together with a statement of the date upon which his original license was issued and the number thereof, which shall be immediately referred to the City Manager. Upon the return of the application with an endorsement thereon by the City Manager that there have been no complaints or convictions against the applicant during the preceding year, the City Clerk shall issue a public driver's license upon the payment of the prescribed fee.
