8-1-2: APPLICATION FOR WATER SERVICE; CONNECTION FEE:
   A.   Application And Agreement For Water Service:
      1.   Any prospective water user of the Village desiring water service shall complete and submit to the utility a "Water Service Application and Agreement". The application and agreement shall request the following information:
         a.   The name and signature of the applicant.
         b.   The address to be supplied by water service.
         c.   The prior address of the prospective water user.
         d.   The desired date for commencement of water service.
      2.   The application and agreement form shall also contain the following: Written acknowledgment by the prospective water user that as the water user, he or she is responsible for the timely and complete payment of all water charges and all installments and surcharges arising from water service supplied to the address identified in the application and agreement form.
   B.   Notice Of Acceptance Or Denial Of Service: The utility shall mail to the applicant a notice stating whether water service shall be supplied to the address requested. If water service shall be supplied, notice shall state the date of its commencement. If utility service shall not be supplied, the notice shall state the reasons why service shall not be supplied.
   C.   Grounds For Denial Of Service: Water service shall not be supplied to any prospective water user if said water user has any outstanding and unpaid water charges and/or installments and surcharges arising from prior water service to this prospective water user and/or has not entered into an agreement with the utility to pay past due amounts.    D.   Connection Fee:
      1.   A water hookup fee of eight hundred fifty dollars ($850.00) shall be paid to the Village at the time of application for service. (Ord. 94-4, 6-20-1994, eff. 8-1-1994; amd. 2019 Code)
      2.   All costs and expenses incident to the installation and connection of Village water shall be borne by the property owner. The property owner shall indemnify the Village from any loss or damage that may directly or indirectly be caused by the installation of the water service.
      3.   All buildings with more than one unit shall have a separate shutoff valve for each unit. (Ord. 94-4, 6-20-1994, eff. 8-1-1994)
      4.   Any person, firm, or corporation violating the provisions of this subsection D shall be fined not less than one hundred dollars ($100.00) and not more than seven hundred fifty dollars ($750.00) for each offense. (Ord. 94-4, 6-20-1994, eff. 8-1-1994; Ord. 17-04, 9-18-2017; amd. 2019 Code)
