(A) Contents of application. Each applicant shall apply in writing to the Building Commissioner. The application prepared by the Building Commissioner shall include the following information, under penalties for perjury:
(1) The applicant’s name/agent, address, telephone number and age;
(2) The applicant’s number of years of experience;
(3) The applicant’s present place of employment;
(4) The applicant is familiar with the provisions of the State Electrical Code, being 675 I.A.C. 17-1.8;
(5) The applicant has not been convicted of a felony crime within the past five years;
(6) Describing all judgments ever entered against the applicant, or of all lawsuits ever settled against them or the firm where they have been employed for an amount in excess of $10,000;
(7) Showing the applicant is currently insured by an insurance policy in an amount of at least $100,000, providing liability coverage for performance of electrical work; and
(8) Including an authorization permitting the Building Commissioner to obtain a complete criminal background check on the applicant.
(B) Rejection of application. The Building Commissioner may reject any application that is incomplete, that demonstrates that the applicant lacks the necessary qualifications to do electrical work, or if the Building Commissioner discovers that the applicant has participated in activities or has been convicted of any crime involving, but not limited to, false statement, fraud, theft, deceit, dishonesty or other moral turpitude.
