§ 117.02 APPLICATION FOR LICENSE.
   Application for a license under this chapter shall be made upon a form prepared by the Village Administrator except that the applicant shall attach an affidavit stating the number of machines. Such license shall be nonassignable and nontransferable and shall apply only to the vending machine for which such license is issued.
