§ 70.24 PERMITS.
   (A)   No person shall operate a golf cart or UTV on a village street without first obtaining a permit as provided herein. Permits shall expire on December 31 of each year. The cost of the permit shall be ten dollars. Applications for a permit shall be made on a form supplied by the village, shall be executed by the applicant, and shall contain the following:
      (1)   Name and address of applicant;
      (2)   Driver's license number;
      (3)   Address and phone number;
      (4)   Name of liability insurance carrier and policy number;
      (5)   The serial number or VIN, make, model and description of golf cart or recreational off-highway vehicle;
      (6)   Waiver of liability on a form provided by the village; and
      (7)   Such other information as the village may require.
   (B)   No permit shall be granted unless the following conditions are met:
      (1)   The vehicle is properly titled with the Illinois Secretary of State, if abled or required;
      (2)   A physically handicapped applicant or operator must submit a certificate signed by a physician, certifying that the applicant is able to safely operate a golf cart or UTV on village streets; and
      (3)   The applicant must provide evidence of insurance in compliance with the provisions of the Illinois statutes regarding minimum liability insurance for passenger motor vehicles to be operated on the roads of the State of Illinois.
   (C)   Permits may be renewed upon demonstration of compliance with the requirements for initial issuance of a permit and the payment of the permit fee.
   (D)   Permits are non-transferable.
   (E)   Permit holders must inform the Channahon Police Department within ten days of any change of address, phone number, and/or insurance coverage.
   (F)   A permit shall be denied for the following reasons:
      (1)   The vehicle is not properly titled with the Illinois Secretary of State Office, if abled or required;
      (2)   Insurance information cannot be verified;
      (3)   Registered owner does not possess a valid driver's license;
      (4)   Owner has provided false information in their application;
      (5)   Vehicle does not conform to the requirements of this subchapter; or
      (6)   Permit holder cannot safely operate golf cart or UTV.
