4-2-6: APPLICATION FOR LICENSE 1 :
   A.   Prior to the issuance of a license, the applicant must submit to the president an application, in writing and under oath, stating the following:
      1.   In the case of an individual, the applicant's name, date of birth, and mailing address.
      2.   The name and address of the applicant's business.
      3.   If applicable, the date of the filing of the "assumed name" of the business with the county clerk.
      4.   In the case of a copartnership, the date of the formation of the partnership, the names, dates of birth, and addresses of all partners, and the persons entitled to a share in the profits of the copartnership. (Ord. 442, 6-2-1999)
      5.   In the case of a corporation for profit, limited liability company, or a club, the date of incorporation or organization, the object for which it was organized, the names and addresses of the officers and directors, and the name of the person who will be managing the establishment for which the license is sought, and if a majority in interest of the stock of such corporation is owned by one person or his nominee, the address and name of such person.       6.   The citizenship of the applicant, his/her place of birth, and if a naturalized citizen, the time and place of his/her naturalization. (Ord. 442, 6-2-1999)
      7.   The character of business of the applicant, and in the case of a corporation or limited liability company, the objects for which it was formed. (Ord. 442, 6-2-1999; amd. 2012 Code)
      8.   The length of time that the applicant has been engaged in the business of that character.
      9.   The location and description of the premises or place of business which is to be operated under such license.
      10.   Whether the applicant has made similar application for a similar other license on premises other than described in the application and the disposition of such application.
      11.   The name and address of the landlord if the premises is leased, and if the premises is leased, the applicant shall attach a copy of the lease agreement to the application.
      12.   The date of the applicant's first request for a state liquor license and whether it was granted, denied, or withdrawn.
      13.   Whether the applicant has made an application for a liquor license which has been denied and, if so, the reasons therefor.
      14.   Whether the applicant has ever had any previous liquor licenses suspended or revoked and, if so, the reasons therefor.
      15.   Whether the applicant has ever been convicted of a gambling offense or felony and, if so, the particulars thereof. (Ord. 442, 6-2-1999)
      16.   In the case of a partnership, sole proprietorship, corporation, or limited liability company, the applicant's name, sex, date of birth, social security number, position and percentage of ownership in the business; and the name, sex, date of birth, social security number, position and percentage of ownership in the business of every sole owner, partner, corporate officer, director, manager and any person who owns five percent (5%) or more of the shares of the applicant business entity or parent corporations of the applicant business entity.       17.   That the applicant will not violate any of the laws of the state of Illinois or the United States, or any of the provisions of this chapter and amendments hereto in the conduct of his place of business.
      18.   In addition to any other requirement of this subsection, an applicant for a license, including an applicant for a special event retailer's license, shall also submit proof of adequate dramshop insurance as specified in section 4-2-9 of this chapter. (Ord. 442, 6-2-1999)
   B.   In the case of a partnership, corporation, or limited liability company, the information and statements required by subsection A of this section shall be furnished as to each partner, and with respect to a corporation or limited liability company, the information and statements required by subsection A of this section shall be furnished as to the president of the corporation and the directors of the corporation, and with respect to the person who is to manage the establishment for which a license is sought.
   C.   If the application is made on behalf of a partnership, firm, association, club, corporation, or limited liability company, then the same shall be signed and sworn to by at least one member of such partnership, firm, association, club or corporation, or by the president and secretary of such partnership, firm, association, club, corporation, or limited liability company.
