§ 111.15  LICENSE REQUIRED; FEE.
   (A)   Each exhibitor, as defined in § 111.01 of this chapter, shall obtain from the town’s Clerk- Treasurer on or before January 1 of each year a license, good from January 1 to the following December 31, for each amusement device or automatic phonograph displayed or in use on any premises under his or her possession, control or ownership, or as soon thereafter as any amusement device or automatic phonograph is installed. This license shall be $1 for each device. The town’s Clerk- Treasurer shall issue no license as required by this section until the fee assessed herein is paid.
   (B)   Each license issued shall contain the following information: name of exhibitor, location of licensed machine, character of licensed machine, name of manufacturer of machine, manufacturer’s serial number of machine, owner of machine and denomination of coin used in the machine.
   (C)   There is levied upon each amusement device licensed by the town and exhibited therein an annual fee of $25, due and payable on the issuance of the annual license by the town’s Clerk-Treasurer.
   (D)   There is levied on each automatic phonograph licensed by the town and exhibited therein an annual fee of $25, due and payable on the issuance of the annual license by the town’s Clerk- Treasurer.
   (E)   The fee on any amusement device or automatic phonograph installed by an exhibitor after July 1 of any year shall be only one-half of the annual rate.
   (F)   The license described in division (A) above shall be displayed publicly for each machine.
