(A) An application for a mobile food unit permit shall be filed, along with the required fee, with the City Clerk. The application shall be made on a form approved by the city and shall contain information requested by the city, including, but not limited to, the following:
(1) A copy of each related license issued by Chisago County or the State of Minnesota Department of Health required to operate a mobile food unit.
(2) A certificate evidencing the insurance coverages required in this chapter.
(3) A site plan that includes a description of the food truck and location on the site where the food truck/trailer will set up.
(B) If applying only for a temporary permit, the applicant must provide the exact dates and locations for each day up to 7 days of proposed mobile food unit operations.
