Every application for a permit shall be made on the form supplied by the city and shall contain the following information:
(A) The name and address of the applicant;
(B) The nature of the applicant's physical disability;
(C) Proof of insurance complying with M.S. § 65B.48;
(D) Certificate of physician complying with the provisions of M.S. § 169.345, subd. 2a, and stating that applicant is capable of safely operating a motorized golf cart if the applicant does not have a valid driver's license.
