§ 111.04 APPLICATION TO ENTER INTO LEASE.
   Any person or entity desiring to enter into a lease with the city for the purpose of constructing or affixing an antenna to city property must first file with the city an application on a form supplied by the city, which application shall contain, but not be limited to, the following information:
   (A)   Name and address of the person or entity; if an entity, a copy of the articles or other such document as will confirm its statutory organization;
   (B)   Description of city structure(s) desired to be leased;
   (C)   Detailed description of the exact nature of the type or mode of wireless transmission to be utilized and the location from and to which any signals will be transmitted or received;
   (D)   List of any known means or modes of wireless transmission known to conflict with or otherwise cause interference with the applicant’s use;
   (E)   A schematic and mechanical diagram, to scale, showing all components of the antenna and other equipment to be placed within or upon the city structure;
   (F)   The hours of operation during which the antenna will be in use;
   (G)   The source of any power needed to operate the antenna and equipment;
   (H)   Whether the applicant will be maintaining the antenna and equipment; if not, the name and address of the person or entity doing the maintenance;
   (I)   The lighting or safety equipment required for the antenna under federal, state and/or local law, regulation or ordinance;
   (J)   Copies of each and every federal, state or local permit or other approval, aside from those of the city, required for the construction, maintenance and/or use of the antenna;
   (K)   Whether the applicant presently anticipates any increase in or change of the use of the antenna; and
   (L)   Any other information required by the city to properly evaluate the request and its impact upon existing and future uses of a structure.
