§ 150.059 REGISTRATION FORM.

   Each registration form shall include the following information:

   (A)   Serial number of the mobile home;

   (B)   Location of the mobile home by legal description and street address;

   (C)   Name, and address, of legal owner(s) of the mobile home; and

   (D)   Date of registration.
