(A) The Board of Health shall charge a fee of $10 for the issuance of a birth certificate; and $10 for the issuance of a death certificate.
(B) The Board of Health shall charge a fee of $5 for each record search.
(C) The Board of Health shall charge a fee of $20 for each of the following: an affidavit of legitimation; a paternity affidavit; and an affidavit requesting amendment.
(D) The Board of Health shall charge a fee of $100 for the issuance of a septic permit.
(E) The Board of Health shall charge a fee of $50 for issuance of an installer certification. This fee must be submitted prior to the issuance or renewal of the certification.
(F) The Board of Health shall charge a fee of $25 for the issuance of an annual pool permit for public swimming pools only.
(G) The Board of Health shall charge a fee of $100 for the issuance of an annual food permit for for-profit establishments. Establishments that sell only prepackaged food and do not engage in food handling shall be charged $50 for the issuance of an annual food permit.
(H) The Board of Health shall charge a fee of $20 per day, with a maximum charge of $100, for the issuance of a temporary food permit.
(I) The Board of Health shall charge a fee of $50 for a late charge if the annual food permit is not paid by January 31.
(J) The Board of Health shall charge a fee of $10 for a TB (Tuberculosis) test. There shall be no charge for tests related to contact investigations.
(K) The Board of Health shall charge a fee of $20 for a flu shot (influenza).
(L) The Board of Health shall charge a fee for “travel immunizations” as follows:

(M) The Board of Health shall charge a fee of $250 annually beginning January 1 of each year for a tattoo business permit.
(N) The Board of Health shall charge a fee of $50 annually for a tattoo artist permit.
(O) The Board of Health shall charge a fee for the following vaccinations, as indicated:

(P) The Board of Health shall charge a fee of $0.10 per page for copying documents kept by the Department.
