§ 153.45 MAINTENANCE PERMIT - SUBMITTAL REQUIREMENTS.
   (A)   Completed maintenance notification form, submitted a minimum of ten business days prior to the start of the project. If it is not possible to submit a form ten business days prior to the start of the project, notification should be provided as soon as possible and the county reserves the right to provide review comments within ten business days of receipt.
   (B)   Sketches, maps, drawings and plans relating to the proposed project.
   (C)   Certain submittal items per § 153.42 may be required.
   (D)   Additional information as relevant and necessary to evaluate an application may be required. Requests for additional information shall be submitted in writing to the responsible party and shall specify requirements for submittal to the county.
