§ 110.06 APPLICATION.
   A person shall submit an application for a license to the Administrator on a form provided by the town. The application shall include the following:
   (A)   The names of the applicant and the business;
   (B)   The name of the business;
   (C)   The street address of the business. If the business has multiple locations, the application shall state the street address of each location; and
   (D)   The mailing address for the business.
