All persons desiring a permit in order to make an opening in any street or sidewalk shall make written application therefor, which application shall show the location of the proposed opening, the purpose therefor and the approximate number of square feet of surface to be cut. The application shall be accompanied by the fee required by this section. The fee required for making any opening in any street or sidewalk shall be according to a fee schedule for the permits, established by the Board of Commissioners, which is hereby adopted as though set out in full herein.
