Anyone desiring a permit under this chapter shall submit an application under oath stating the exact location of the proposed well location or installation, the distance to any structures on adjacent premises and the type of structures involved, the exact nature of the operation for which permit is requested, including pits, tanks, etc., the area unitized, or intended to be unitized and which will be served by the well, the amount of the area under lease to the applicant, and the term of the lease, and the name and address of the applicant, operator or other persons who will be responsible for the operation. A one hundred dollar ($100.00) fee will be paid the city clerk at the time of the filing of the application. (Ord. 762, 9-6-1960)
