905.02 PERMIT APPLICATION.
   (a)   Persons submitting applications to excavate and/or open a street must pay the required permit fee(s) and submit a maintenance bond in an amount not less than Ten Thousand Dollars ($10,000) naming the Village as an obligee. Said maintenance bond must be valid for a period of not less than two (2) years.
   (b)   All applications for permits shall give the precise location of the property, number of the lot, name of the street, name of the owner and name of the licensed excavator, sewer tapper and sewer builder employed to do the work, and shall be filed with the Village at least twenty-four (24) hours prior to the opening of the trench or commencing the work.
   (c)   No permit shall be deemed to authorize anything not stated in the application. For any misrepresentation in such application, the permit of the licensed excavator, sewer tapper or sewer builder shall be suspended. If such misrepresentation appears to be willful, said licensed individual will not be allowed to perform any work in the Village of Cardington for the immediately succeeding twelve (12) months.
   (d)   The opening and restoration of a pavement or other surface shall be performed under the direction and to the satisfaction of the Administrator and in accordance with rules, regulations and specifications established by the Village of Cardington.
